Triclosan을 배합한 세치제의 치은염완화효과와 치면세균막형성억제효과 및 구강질환원인균에 대한 항균효과에 관한 연구

Abstract
결론
저자들은 0.2% triclosan을 배합한 세치제의 치면세균막형성억제효과와 치은염완화효과를
검토하고자, 치주병이나 치아우식증이 심하지 않은 20대의 남녀 62명을 실험대상자로 선정
하여, 이들을 Quigley와 Hein의 치면세균막평점기준과 Loe와 Sillness의 치은염평점기준에
의거한 기초검사후 두군이 유사한 치은염발생과 치면세균막형성정도를 나타낼 수 있도록 실
험군과 대조군으로 각각 31명씩 배정하였고, 이들에게 0.2% triclosan이 배합된 실험용세치
제와 triclosan을 제외한 모든 성분이 동일한 대조세치제로 6주간 잇솔질하게 한 후, 잇솔질
기간 4주와 6주경과후 재차 치면세균막지수와 치은지수를 측정하여, 최종적으로 실험을 완
료한 28명의 실험군과, 24명의 대조군의 실험세치제사용군별 사용기간별 치은지수와 치면세
균막지수를 산출 비교 검토함과 아울러, 0.2% triclosan을 배합한 세치제의 다섯가지 구강질
환원인균에 대한 항균효과를 세균발육억제최대 희석 비율(maximum inhibitory dilution,
MID)측정법과 원판확산(Paper Disk)측정법을 통하여 확인 검토한 결과, 다음과 같은 결론
을 얻었다.
1. 0.2% triclosan 배합세치제사용군과 대조세치제사용군의 실험시작전 치은지수에 대한
각군의 실험시작후 4주와 6주 경과후 치은지수를 Paired t-test한 결과, 실험세치제사용군에
서 실험기간이 경과함에 따라 점차적으로 감소하였고, 통계적으로 유의한 차이가 인정되었
으나(P(0.05), 대조세치제사용군에서 통계적으로 유의한 차이가 인정되지 않았다(P>0.05).
2. 0.2% triclosan배합세치제사용군에서 사용기간이 증가함에 따라 치면세균막지수가 감소
하였으며, 치면세균막형성억제효과는 실험세치제사용후 4주경과 이후부터 대조세치제사용군
에 비하여 통계적으로 유의하게 나타났다(P<0.05).
3. 0.2% triclosan배합세치제와 대조세치제는 공히 Streptococcus muttons, Streptococcus
sanguis, Lactobacillus acidophilus, Actinomyces viscosus, Fusobacterium nucleatum등 치
아우식증원인균, 치주조직병원인균 및 구취유발원인균에 대한 항균효과가 있었으며, 0.2%
triclosan배합세치제는 대조세치제에 비하여 Lactobacillus acidophilus와 Actinomyces
viscosus에 대한 보다 강한 항균효과를 나타내었다.
A total of 62 adult male and female subjects began a 6-week, double-blind clinical
study comparing the effect on gingivitis and plaque formation of a 0.2%
triclosan-containing dentifrice to a placebo dentifrice. Placebo dentifrice had the same
formulation with experimental dentifrice except it did not contain 0.2% triclosan.
Subjects were divided by two balanced groups according to baseline plaque and
gingivitis scores. Then, they were assigned to use either the triclosan-containing
dentifrice or the placebo dentifrice for the next 6 weeks. Finally, 28 experimental and 24
control subjects completed 6 weeks of the study.
Plaque index and gingival index were scored 3 times : before the study, after 4 weeks
of use of each dentifrice, after 8 weeks of use of each dentifrice. Quigley-Hein Plaque
index and Loe-Silness Gingival index were used to score each index.
Also, the antimicrobial erects of a 0.2% triclosan-containing dentifrice against the
pathogens of oral diseases such as Streptococcus mutans, Streptococcus sanguis,
Lactobacillus acidophilus, Actinomyces viscosus, Fusobacterium nucleatum were
evaluated with the agar dilution and paper disc methods.
The results obtained were as follows:
1. After 6 weeks, the triclosan-containing dentifrice was shown to provide a 20.4%
reduction in gingivitis, as compared to the placebo dentifrice, but not statistically
significant(P>0.05).
2. After 6 weeks, the triclosan-containing dentifrice was shown to provide a
statistically significant reduction in plaque formation, as compared to the placebo
dentifrice(P<0.05).
3. The triclosan-containing dentifrice showed more effective antimicrobial activities on
Lactobacillus acidophilus, Actinomyces viscosus, as compared to the placebo dentifrice.
